New Patient Registration

Joining our list is very simple

We accept patients who live within our practice area, please check our boundary using the map below. You may register with either the main surgery in Leominster or with the branch surgery in Bodenham. If you are a registered patient who moves out of our boundary we may consider a request to stay with us, but this is at the discretion of the practice and is generally only considered for patients who commute to work in the area. If you are accepted as an "out of area" patient it will only be on the basis that you do not receive home visits.

Although we generally receive your electronic medical records within a day or two, this isn’t always the case, so please do make sure you have sufficient supplies of any regular medication from your previous practice.

Temporary Residents

If you require urgent treatment whilst staying in our area you can be registered as a temporary patient for up to three months. You will still remain on your own Practice's list during this time. After three months you will be removed from our patient list and will need to either register as a permanent patient with us or return to your own practice.

GP2GP

Ryeland Surgery uses GP2GP. This means that, as long as your previous surgery also uses GP2GP, your electronic medical records will be automatically up loaded onto our clinical system. This helps improve patient care as it means that your new GP will usually have access to a full and detailed medical record from the date you register with our practice. Similarly, if you register with another GP practice that uses GP2GP, your medical record will be transferred electronically to your new GP. Your paper medical record will continue to be sent to your new GP via Primary Care Support England, which can take up to three months.

Named Accountable GP

All patients (including children) registered at Ryeland Surgery have a named, accountable GP who is responsible for their overall care at the practice.

If you wish to know the name of your usual GP, please contact us or ask the Patient Service Advisor next time you visit the surgery or check on the NHS App. If you are prescribed medication, you can find the name of your usual GP at the bottom of your prescription.

Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and, if your needs are urgent, you may need to discuss them with an alternative doctor.
